Media Release Form

Bethany strives to honor and protect our clients’ stories. Whenever clients contribute to a story, we ask them to complete a media release form. This provides a written record that both Bethany and the client understand how stories will be shared, and we're aware of any use restrictions.   Instructions:

1. Enter your name and email address in the fields on this screen
2. You will receive an email with a link to our media release form
3. Complete the form online and click submit
4. You will receive a digital copy of the form, and Bethany will receive a copy to store with your story
